***   8-Week Online Mindfulness and Compassion Course ***  ***  September - November 2020  ***

    Welcome to the Course.

     Collecting this information is to ensure that the course will be helpful for you at this particular time in your life and that any ongoing challenges you face can be accommodated safely and appropriately over the eight-week period.
    ​What happens next?
    After I have received your online form I will arrange a mutually convenient time to talk to you and for you  to ask any questions that you may have. I may also ask you any questions that I have in relation to the information that you have provided on the form. Please be aware that all applications (including submission of the online application form, payment and the 1-1 call, after which your place is reserved on the course) must have been concluded by 5pm on Wednesday 9th September 2020. 
    ​What about confidentiality?
    The information that you provide on this form will be kept confidential. For more about confidentiality and other professional standards please see here.

    Mental Well being
    Please let me know in the box below if you have currently, or have had in the past, any problems with your mental well being. This might include, but is not restricted to, difficulties with depression, anxiety, stress, trauma or psychosis. Please also let me know if you have been bereaved in the past year or so. 

    Physical Wellbeing
    Please let me know in the box below if you have currently, or have had in the past, any problems with your physical wellbeing. It is also helpful for me to know if you have a long-term health condition that might make sitting, standing or taking part in simple stretching exercises difficult for you. Whilst the course is online there will be some simple mindful movement included in the teaching.
